Living Wills & Health Care Power of Attorney

Advance health care directives or advance directives are instructions on what actions should be taken for your health in the event that you are no longer able to make decisions due to incapacity or illness.  A Living Will is one form of advance directive, leaving instructions for treatment. 

Another form authorizes a specific type of power of attorney or health care proxy, where someone is appointed to make decisions on your behalf in case you become incapacitated.  You may also have a combination of living will, power of attorney or health care proxy.
